The High Risk Nurse Care Manager provides nursing services to patients in accordance with all applicable Massachusetts State Laws and Department of Public Health regulations. The Nurse Care Manager is responsible and accountable for coordinating the care of a specified group of patients by developing, implementing, and evaluating individualized patient care plans, performs case management services taking into consideration the patient’s physical, cultural, social and psychological needs, follows the patient through health care continuum, including acting as liaison for network staff, hospital staff, PCP and other healthcare team members. Position is located at the Neponset Health Center.

Minimum Skills, Experience and Educational Requirements: 3 years of nursing experience in acute care, ambulatory care or health insurance environment, strong analytical and clinical problem solving skills, experience in using care plans and developing interventions, experience with Microsoft Office applications, strong oral and written communication skills.

Required Certifications and Licenses: Current MA State License as Registered Nurse, Professional certification as a Case Manager required within one year of employment.
Substitutions for Skills, Experience and Education: BS in Nursing can be substituted for one year of experience.
Preferred Skills, Experience and Educational Requirements: BSN, Clinical Care Management (CCM) Certification, years of related experience in ambulatory care environment, NextGen Experience, working knowledge of NCQA patient centered medical standards, GE Lean Principles.
