This position is to be held by a registered nurse. This position is responsible for MDS assessments and input of annual, quarterly and change of condition as assigned by MDS coordinator.

RESPONSIBILITIES

ESSENTIAL

1. This person receives work directives from the MDS Coordinator.The RN is further responsible for completing the assignments and reporting back to the MDS Coordinator.
2. Review policies and procedures relating to MDS, transmission, care planning and participate in making recommended changes.
3. Ensure that incomplete records/charts are returned to appropriate department for correction in relation to MDS/care planning process.
4. Other duties as assigned.

QUALIFICATIONS

1. Strong physical assessment skills and knowledge.
2. Ability to work in a partnership environment, which fosters effective teamwork in meeting the mission of Oakwood.Facilitates work flow during absence of MDS Coordinator.
3. Genuine interest and liking of older people.
4. Ability to handle confidential information.
5. Ability to learn, understands, and follows Oakwood policies and procedures.
6. Fluent in the pathophysiology of common clinical problems in a geriatric population and the treatment modalities and the standards of practice associated with various treatment modalities.
7. Ability to plan, organize, prioritize work, and be accurate in job details.
8. Participates in Medicare and managed care case-management team.
9. Ability to assume responsibilities without direct supervision.
10. Basic computer knowledge.
11. Work flexible hours as needed.
12. Follow organizational/departmental policies and procedures.

EXPERIENCE AND/OR TRAINING

1.Currently licensed RN within the State of Wisconsin.

2.BSN desirable.

3.Minimum of 2 years experience working with MDS desirable.

4.Minimum of 2 years experience in long term care rehabilitation nursing desirable.
