• FULL TIME- Salaried

• WEEKEND PHYSICAL THERAPIST – Special Rates Apply ($100 visit; 150 Oasis SOC)

Aspen is hiring for this position in Denton & Dallas Counties. Experience preferred but willing to train.

Qualifications for this position:

• Current Texas PT License.
• Must have a minimum of 2 years therapy experience in Home Health.
• Excellent communication/documentation skills.
• Requires walking, pushing, pulling, and lifting up to 40 pounds

Responsibilities:

• Perform initial and ongoing clinical assessments to identify patient needs.
• Determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health.
• Develop and revise a Plan of Care in consultation with the physician and other care-team members.
• Self-directing with the ability to work with little direct supervision.
• Demonstrate effective oral and written communication skills.
• Ensures confidentiality and security of the client’s medical information.
• Provide skilled therapy care to patients utilizing infection control measures.
• Complete appropriate documentation in a timely manner.
