Position Summary:

Promotes the health and wellness of the individuals supported. This includes services, advocacy, and education to contribute to continued provision of quality health care to persons with developmental disabilities and neurological impairments within the specific program. Serves as key liaison, advocate, and a resource of information regarding both routine and special healthcare needs to support a comprehensive care plan for each person.

Essential Responsibilities:

  • Identifies and assesses the routine, special, and changing healthcare needs of individuals supported and works jointly with the healthcare and program team to implement a comprehensive healthcare plan and service delivery system that is responsive to the needs of persons served, emphasizes the value of integrated healthcare disciplines and services by linking individuals with appropriate providers, and reduces gaps in or overlap of services;
  • Dependent upon program needs, may attend or provide input to the Interdisciplinary Team (IDT) or Trans-Disciplinary Team (TDT) process, attend weekly rounds and/or other team/departmental meetings, etc., to present significant findings of health history, physical assessment, reports of medical consultants, etc. When requested, provides information pertinent to the case of each individuals supported and participates in discussions regarding the needs and progress of individuals and the maintenance or modification of services;
  • Monthly on-site visits of program sites to promote the health, safety, security, and overall quality of care for individuals supported. Identifies existing and potential issues of concern and takes appropriate action to address, resolve, and avoid reoccurrence of issues; Reviews individuals records as needed or recommended by program leadership; makes recommendation on medical care that may be needed for the individual and communicates with program any outstanding annual licensing requirements.

Education & Experience:

Registered Nurse required with a current and valid P.A. license. Degree in Nursing required. Minimum of one year of general nursing experience required, with two years of prior experience preferred.

Special Skills:

Knowledge of case management services and medical assessment procedures a plus, as well as knowledge of nursing/clinical operations and procedures and primary care principles and practices. Ability to work independently and to observe, assess, and record individual medical symptoms, reactions, and progress required. Effective communication and interpersonal skills to interact regularly with various internal and external agencies, healthcare providers, specialists, consultants, direct care staff, etc., as well as effective organization and time management skills required. Working knowledge of computer and related computer software preferred. Valid driver’s license required in incumbent’s legal name and current address with no provisional restrictions. Minimum 18 years of age required.
