EXCELLENT OPPORTUNITY TO PRACTICE REWARDING NURSING IN AN ADULT DAY HEALTH SETTING! 

Monday through Friday, Daytime hours, no weekends.

The Center Nurse is responsible for the implementation, assessment, delivery, supervision, evaluation, and coordination of nursing care based on Elder Service Plan participant’s care plan. (The Elder Service Plan is a Program for All Inclusive Care for the Elderly.)

The Nurse performs direct care/triage of participant issues and initial evaluation of episodic complaints and refers to the Providers as appropriate, teaches, counsels, & supervises the participants & caregivers regarding nursing care needs and related problems and issues regarding the participant’s home situation, works with nursing assistants regarding personal care needs of center clients, and participates actively as a member of Interdisciplinary team meetings.

Qualifications: Current MA RN license, 2 years of nursing experience with adult population of which one year must be with a geriatric population, strong physical assessment and organizational skills, ability to work in a team environment.

The Elder Service Plan, a PACE program, is an innovative, comprehensive program designed to maintain frail elders in their community by managing their primary, sub-acute and long term care needs. The Elder Service Plan is located at 1135 Morton Street in Mattapan, MA.
