The Silvercrest Center for Nursing and Rehabilitation is a unique center of excellence, dedicated to caring for clinically complex patients. Our 320 bed Skilled Nursing Facility and 80 apartment Senior Housing Campus are located on a four acre site in residential Briarwood, Queens, which is easily accessible by major highways and public transportation. Silvercrest is a regional leader in post-acute and rehabilitation services. Today Silvercrest houses the largest non-hospital pulmonary/ventilator care program in New York State.

The ideal candidate will have prior MDS/Case Management experience, preferably in a Pulmonary/Sub-Acute environment.

BS required/MS preferred. MDS Certification preferred.
